The 30-second report, in order

  1. Who you are: your unit and your level (BLS or ALS), and the hospital you're calling. Hail them first or lead with your unit; either way, this comes before anything about the patient.
  2. How you're coming: emergent or non-emergent, and your ETA in minutes.
  3. Who you have: age and sex.
  4. Why: the chief complaint, or the mechanism for trauma.
  5. What you found: two or three pertinent findings, with the history that matters.
  6. Mental status: alert and oriented times four, or the GCS.
  7. Vitals: blood pressure, heart rate, respirations, oxygen saturation; blood sugar when it's the story.
  8. What you did: every treatment, with device, dose or flow.
  9. What happened: the patient's response to it.
  10. Paramedic: the order. When you need medical control, ask after the response: "requesting orders for" the drug, the dose and the route. Then listen for granted or denied, and read it back.
  11. Close: ETA again, or "any questions?"

This is the order the NHTSA EMT Instructional Guidelines teach for the verbal report and the one most receiving hospitals want to hear. Your agency's format wins; this drill grades the elements and the order, and times you against 30 seconds, the length a busy charge nurse will actually listen to.

Your voice stays on your device: the page only uses speech recognition that runs on the device itself (Chrome or Edge with on-device recognition, or the app). Anywhere else you give the report out loud, then check it yourself or type it. Nothing is recorded or sent anywhere. Dictation writes numbers as numerals, so "one ten over seventy" and "110/70" grade the same.
