How to get a PT or PTA license in Oklahoma

In Oklahoma, physical therapists and PT assistants are licensed by the State Board of Medical Licensure and Supervision, advised by its Physical Therapy Committee. Everything below comes from the board's own pages and rules, checked October 18, 2026. Fees and rules change, so confirm on the linked pages before you pay.

Oklahoma, step by step

Oklahoma State Board of Medical Licensure and Supervision (Physical Therapy Committee)

  1. Apply to the board with the $100 application processing fee; the license fee is $50 for a PT and $35 for a PTA. You need a CAPTE-accredited program.
  2. Background check: sign the board's Extended Background Check consent (criminal history check under the Fair Credit Reporting Act).
  3. The NPTE: you are admitted only once the board has your qualifications, and you must test within 60 days of the written notice of eligibility (OAC 435:20-3-5).
  4. If you fail: you may retake once without reapplying, within six months, and may work under a licensed PT's direct, on-the-premises supervision for up to six months. After a second failure you may not practice and must meet with the Committee and reapply; after a third you cannot reapply to test in Oklahoma.
  5. Keep it: renewal is yearly ($50 PT, $35 PTA). Oklahoma is in the PT Compact.

The national steps, briefly

Every state licenses physical therapists and PT assistants on the NPTE, with the same passing score everywhere (600 on a 200-800 scale): a CAPTE-accredited degree, your state board's application, then NPTE registration with FSBPT ($485 plus Prometric's sitting fee), which your board approves before FSBPT sends your Authorization to Test. The PT exam is 225 questions in 5 hours and the PTA exam 180 questions in 4 hours, both on fixed dates four times a year; you may test 3 times in 12 months and 6 times in a lifetime (FSBPT NPTE Candidate Handbook).
